A standardized request from a prospective landlord or property manager to a lease applicant's previous landlord for the applicant's rental history, sent with the applicant's signed authorization (page 4 of TXR 2003). Twelve questions cover notice of non-renewal, lease dates and rent, timely payment, returned payments, money owed, damage, animals, lease violations, criminal record, whether the landlord would lease to the applicant again, early termination, and other relevant information. No signature line: the previous landlord completes, names and dates the form and returns it to the requesting party.
