TXR 2003Residential Lease Application

Prospective tenant's application for a residential lease: applicant identification, property condition acknowledgement, representation and marketing source, current and previous housing and employment, other income, emergency contact, occupants, vehicles, animals (with assistance-animal designation), yes/no disclosure questions, the authorization for credit / criminal / rental / employment verification, the application fee and deposit, and the page 5 Authorization to Release Information. Each occupant and co-applicant 18 or older submits a separate application. The applicant signs pages 4 and 5; the landlord's side records the decision in the 'For Landlord's Use' box.

Form Outline

The TXR 2003 form is organized into the following sections:

  1. Property and Lease Terms
    Identifies the rental property, the anticipated move-in date, the proposed monthly rent and security deposit, and the initial lease term requested (months). The property address repeats in the running header of pages 2–5.
  2. A. Applicant Identification
    The applicant's legal name (as listed on proof of identification), former last name, contact details, text-message consent, Social Security number, driver license / ID number and state, date of birth, physical description, and any co-applicants (each co-applicant 18 or older must submit a separate application).
  3. B. Property Condition
    Whether the applicant has viewed the Property in-person prior to submitting this application (strongly encouraged). Landlord makes no express or implied warranties as to the Property's condition; the applicant may request repairs or treatments for Landlord to consider should the parties enter into a lease.
  4. C. Representation and Marketing
    Whether the applicant is represented by a REALTOR® or other agent (and that agent's name, company, e-mail and phone), and how the applicant was made aware of the Property (sign, internet, other).
  5. D. Applicant Information
    Housing (current and previous address with landlord or property manager contact, move-in / move-out dates, rent and reason for move), employment and other income (current and previous employer with verification contact, dates, gross monthly income and position; other income to be considered), and an emergency contact who is not an occupant or co-applicant.
  6. E. Occupant Information
    All other persons that are not co-applicants who will occupy the Property: name, relationship to the applicant, and date of birth. Four rows on the form.
  7. F. Vehicle Information
    All vehicles to be parked on the Property (cars, trucks, boats, trailers, motorcycles, other types of vehicles): type, year, make, model, license plate number / state and monthly payment. Six rows on the form. State, local, and/or HOA ordinances may restrict parking certain vehicles on the Property.
  8. G. Animals
    Whether any animals (dogs, cats, birds, reptiles, fish, other types of animals) will be kept on the Property and, if yes, each animal's type and breed, name, color, weight, age in years, gender, and Y/N for neutered, bite history, rabies shots current and assistance animal. Four rows on the form. Assistance animals need appropriate documentation with a reasonable accommodation request.
  9. H. Additional Information
    Yes/No questions: waterbeds or water-filled furniture; smoking or vaping; renter's insurance; military status and orders limiting the stay to one year or less; and whether Applicant has ever been evicted, been asked to move out, breached a lease, filed for bankruptcy, lost property in a foreclosure, been convicted of a crime, whether any occupant is a registered sex offender, credit problems, and whether there is additional information to be considered — with an 'Additional comments' blank for the details.
  10. I. Authorization
    Applicant authorizes Landlord and Landlord's agent to obtain a credit report, a criminal background check, and to verify rental / employment history; notice of Landlord's right to continue to show the Property; privacy policy; the non-refundable application fee and who it is payable to; whether Applicant submits an application deposit; the Property Code Chapter 5, Subchapter H notice; and the acknowledgement and representation the applicant signs. The fee, payee and deposit amount are set by the landlord's side; the applicant checks submits / will not submit and signs.
  11. Authorization to Release Information (page 5)
    The applicant's standalone authorization: "I, ____ (Applicant), have submitted an application to lease a property located at ____. The landlord, broker, or landlord's representative is: ____" — permitting current and former employers, landlords, mortgage lenders and financial institutions to release information to the named person, and the named person to obtain a consumer report and background information. The applicant signs and dates.
  12. Received stamp and For Landlord's Use
    Completed by the Landlord or Landlord's agent, not the applicant: the "Received on ____ (date) at ____ (time)" stamp at the top of page 1, and the page 4 box recording on what date, who (name/initials) notified ☐ Applicant ☐ ____ by ☐ phone ☐ mail ☐ e-mail ☐ in person that Applicant was ☐ approved ☐ not approved, with the reason for disapproval. Leave blank when preparing the application.
