TXR 2225Response to Request for Assistance Animal

One-page landlord response to a tenant's or applicant's request for a reasonable accommodation for an assistance animal. It states the landlord's existing animal restrictions on the property (all animals prohibited, only certain animals allowed, breed/size/weight limits, animal deposit or fee, other), acknowledges the request to modify or make an exception to those restrictions, informs the tenant that the landlord may request reliable documentation of the disability or disability-related need when it is not readily apparent or known, and commits the landlord to evaluate the request under fair housing laws and respond promptly. The tenant completes the description of the assistance animal and signs an acknowledgment of receipt.

The TXR 2225 form is organized into the following sections:

  1. Header and Addressing
    "To: ___ (Applicant/Tenant)", "From: ___ (Landlord)" and "Concerning the Property at ___".
  2. A. Landlord's Conditions or Restrictions on Animals
    "Landlord imposes the following conditions or restrictions on animals on the Property: (Check all that apply.)" — five boxes; three of them have a description line.
  3. B. Request Acknowledged
    Printed text: "You have requested Landlord modify or provide an exception to the above restriction(s) for the assistance animal described below." No blanks.
  4. C. Documentation
    Printed text: if either the disability or the disability-related need for the assistance animal is not readily apparent or known to Landlord, Landlord may request reliable documentation of it. No blanks.
  5. D. Evaluation
    Printed text: "Landlord will evaluate your request in accordance with fair housing laws and will respond promptly." No blanks.
  6. Landlord Signature
    "This form was provided by:" (no blank on this revision), then the "Landlord … Date" rule and, beneath "Or signed for Landlord under written property management agreement or power of attorney", the "By: … Date" rule — all e-sign tabs.
  7. Description of Assistance Animal
    The boxed panel: "Applicant/Tenant: Please complete the following information, then sign and return to Landlord." — Name, Type, Breed, Color, Weight, Age (in years), Gender, and Yes/No for Neutered/Spayed, Declawed, Rabies Shot Current and Bite History. The applicant/tenant answers this section.
  8. Applicant/Tenant Acknowledgment
    "By signing below, I acknowledge that I received, read, and understand this information." — the "Applicant/Tenant … Date" rule (e-sign tab).
