Form Outline

The TXR 2225 form is organized into the following sections:

  1. Header and Addressing
    Standard addressing block identifying the parties (tenant/applicant and landlord) and the rental property address.
  2. Animal Conditions and Restrictions
    Documents the landlord's current animal policy on the property. The landlord checks all applicable restrictions including total prohibition, species limitations, breed/size/weight restrictions, deposit or fee requirements, and any other conditions.
  3. Accommodation Request Acknowledgment
    Acknowledges that the tenant or applicant has formally requested the landlord modify or waive animal restrictions for an assistance animal, which is described in the animal description section of the form.
  4. Documentation Request Rights
    Informs the tenant that the landlord may request documentation of the disability or the disability-related need for the animal, but only when such disability or need is not readily apparent or already known to the landlord.
  5. Evaluation Commitment
    Commits the landlord to evaluate the assistance animal request in compliance with federal and state fair housing laws and to respond to the tenant promptly.
  6. Landlord Signature and Provider
    Signature block for the landlord (or authorized representative under a written property management agreement or power of attorney) with date and identification of who provided the form.
  7. Description of Assistance Animal
    Detailed information section completed by the tenant describing the assistance animal's identifying characteristics and health/behavior history. Includes name, type, breed, color, weight, age, gender, spay/neuter status, declaw status, rabies vaccination currency, and bite history.
  8. Tenant Acknowledgment and Signature
    The tenant or applicant signs and dates the form acknowledging they received, read, and understand the information provided.
