  5. Ownership Changes Notification
    Allows the owner to formally notify the broker of any changes affecting property ownership, such as transfers, sales, inheritance, or entity restructuring.
  6. Execution and Signatures
    Signature section where the broker's associate and the owner(s) sign and date the amendment to make it effective.
