A standardized request from a prospective landlord or property manager to a lease applicant's employer to verify employment, sent with the applicant's signed authorization (page 4 of TXR 2003). The employer is asked for the applicant's beginning date of employment, monthly gross income, current position and any other relevant information, and the employer's representative titles, signs, prints and dates the completed form and returns it to the requesting party by phone, fax or e-mail contact given.
