This form provides the tenant's formal written notice to the landlord of the tenant's intention to vacate the rental property. It serves two purposes: (1) standard end-of-lease notice of intent to vacate on or before the lease end date, and (2) early termination notice under special statutory provisions of the lease (Paragraph 25; Paragraph 28 for leases dated on or before January 4, 2026) — military transfer/deployment, family violence, or sexual offenses/stalking — with the required documentation attached. It lists seven move-out reminders (keys and access devices, forwarding address, clean condition, move-out date changes, other requirements, holdover rent, the landlord's right to show the property), the tenant's acknowledgment that the lease obligations continue until termination, and an optional Landlord's Acknowledgment of Receipt.
