It captures the date, time, and description of the incident, which officials were called (police, fire department, EMS, other) and by whom, where the official report may be obtained and the attending official's name, injured persons and their treatment (facility, who reported the injuries, the injuries), property damage, witness names/addresses/phone numbers, the action taken, the attachments (police reports, photographs, witness statements, repair estimates, other), and who completed the report and when.
