This form provides a standardized template for documenting an incident that occurred on a managed property, such as a crime, death, personal injury, or property damage. It captures the date, time, and description of the incident, which officials were called (police, fire department, EMS, other) and by whom, where the official report may be obtained and the attending official's name, injured persons and their treatment (facility, who reported the injuries, the injuries), property damage, witness names/addresses/phone numbers, the action taken, the attachments (police reports, photographs, witness statements, repair estimates, other), and who completed the report and when.

The TXR 2215 form is organized into the following sections:

  1. Property Identification and Incident Overview
    "Re: Property at ___" then "On ___ (date) at ___ (time) the following incident occurred on the Property (describe incident such as crime, death, personal injury): ___" (five lines on the paper).
  2. Officials Called
    "The following officials were called to the Property: ❑ Police ❑ Fire Department ❑ EMS ❑ ___. If called, they were called by ___. A report from the official(s) called to the Property may be obtained from ___. The attending official's name is ___."
  3. Injured Persons
    "The following persons were injured ___ and were taken to ___ for medical treatment. It was reported by ___ that their injuries consisted of ___" (four lines).
  4. Property Damage
    "The Property sustained the following damage: ___" (five lines).
  5. Witnesses
    "The names, addresses, and phone numbers of witnesses are: ___" (five lines).
  6. Action Taken and Attachments
    "The following action was taken: ___" (five lines). "Attached are the following: ❑ Police reports ❑ Photographs ❑ Witness statements ❑ Repair estimates ❑ Other: ___".
  7. Completed By
    "Completed by: ___ … Date" — the person completing the report (property manager, broker or associate) signs the rule; "Date" is printed under its right end. Placed as an e-sign tab when the document is sent.
