The TXR 2208 form is organized into the following sections:

  1. Header / Identification
    Identifies the tenant(s), landlord, and the property address that is the subject of the lease and this notice.
  2. Reason for Breach
    Specifies the reason the tenant has breached the lease, either nonpayment of rent or another described lease violation.
  3. Termination Effective Date
    Sets the date on which the tenant's right to occupy the property is terminated, either as a number of days after delivery or a specific calendar date.
  4. Demand to Vacate
    Formal demand that the tenant vacate by the termination date specified in Paragraph B.
  5. Reservation of Remedies
    Preserves the landlord's right to pursue any and all legal remedies for the tenant's breach, including damages, attorney's fees, and court costs.
  6. Signature and Authorization
    Signature block for the landlord or authorized property manager/attorney-in-fact to sign and date the notice.
  7. Means of Delivery
    Documents how the notice was delivered to the tenant, including the method, date, and any tracking information.
