Form Outline

The TXR 2015 form is organized into the following sections:

  1. Property
    "ADDENDUM TO RESIDENTIAL LEASE CONCERNING THE PROPERTY AT" — the two-line address blank under the title. The bold disclaimer under it (a disclosure of Landlord's knowledge as of the date signed, not a warranty) has no blank.
  2. A. 100-YEAR FLOODPLAIN
    "Landlord ❑ is or ❑ is not aware that the dwelling you are renting is located in a 100-year floodplain." If neither box is checked, the tenant should assume the dwelling is in a 100-year floodplain. Advises tenants that properties outside floodplains may still flood, that FEMA has free online flood maps, and that most tenant insurance does NOT cover flood damage.
  3. B. DAMAGE TO A DWELLING DUE TO FLOODING DURING THE LAST FIVE-YEAR PERIOD
    "Landlord ❑ is or ❑ is not aware that the dwelling you are renting has flooded at least once within the last five years."
  4. Definitions
    Provides statutory definitions for '100-year floodplain' (1% or greater annual chance of flooding per FEMA) and 'flooding' (inundation from overflow of waters, unusual runoff/surface water accumulation, or excessive rainfall). Notes the elevation exemption for the floodplain disclosure. No blanks.
  5. Tenant Acknowledgement
    "The undersigned Tenant acknowledges receipt of the foregoing notice." No blanks.
  6. Signatures
    Signature lines for up to two Landlords and up to four Tenants, each with a Date.
