The TXR 1957 form is organized into the following sections:

  1. Header and Property Identification
    Header block identifying the declaration, legal authority, property details, and the survey date being relied upon.
  2. 1–3. Declarant Identity and Transaction Context
    Establishes the declarant's basis for knowledge about the property and the context of the title insurance transaction. Printed text — no blank.
  3. 4. No Changes Since Survey Date
    The core certification: the declarant states there have been no changes to the property since the survey date in four specific categories -- "EXCEPT for the following (If None, Insert “None” Below):" two ruled lines. This is the declarant's own statement.
  4. 5–7. Liability Limitations and Truthfulness
    Limits the declaration's scope and the declarant's liability, and includes the truthfulness certification with warning about false statements. Printed text — no blank (¶6 and ¶7 are on page 2).
  5. Perjury Declaration and Execution
    Page 2, two side-by-side boxes (left = Declarant 1, right = Declarant 2): "My name is ____. My date of birth is ____. and my address is ____ ____. I declare under penalty of perjury that the foregoing is true and correct. Executed in ____ County, State of ____, on the ____ day of ____, ____. Signed: ____ / Declarant". Each declarant's own statement under Texas CPRC § 132.001; the agent may transcribe.
