This form authorizes a named person or entity to obtain one copy of the consumer's credit report (consumer report) from specified consumer reporting agencies. It complies with the Fair Credit Reporting Act by obtaining written consumer consent, explaining how the report may be used, clarifying that the authorized party may not give the consumer a copy or reveal its contents, releasing the authorized party from liability, and setting an expiration date for the authorization. Up to two consumers sign.
