Form Outline

The TXR 1413 form is organized into the following sections:

  1. TO: (Mortgage Company)
    Six lines under "TO:": (Mortgage Company), (Address), (City, State, Zip), (phone) (fax), and (E-mail).
  2. FROM: (Seller)
    One line: the seller (borrower) requesting the mortgage information.
  3. RE: Request for mortgage information concerning Loan No. ____ secured by the Property at ____
    Identifies the loan number and the property address, and gives formal notice of the seller's intent to sell with a possible pay-off or assumption.
  4. (1) Loan Information
    Requests the original loan amount, date of the original loan, original term, type of loan, lien priority, current principal balance, annual interest rate, and reserve account balance. No blanks.
  5. (2) Payment Information
    Requests the next payment date, total payment, frequency of payment, principal and interest payment, taxes and insurance escrow payment, and mortgage insurance premium. No blanks.
  6. (3) Prepayment Information
    Requests the amount of any prepayment penalty and any applicable waiver. No blanks.
  7. (4) Assumption Information
    Requests the amount of any transfer fee, whether buyer qualification is required, any interest rate escalation, and any change in the payment. No blanks.
  8. (5) Insurance Information
    Requests the types of insurance, premium amounts, premium periods, and the agent's name and contact information. No blanks.
  9. (6) Tax Information
    Requests the amount of annual taxes last paid and the year taxes were last paid, itemized by city, school, county, and other taxing authorities. No blanks.
  10. (7) Other Information
    "any other information you believe is relevant." No blanks.
  11. Please return the mortgage information to
    The recipient's name with a "(❑ Broker ❑ Owner)" box pair, then "Attn:", (Address), (City, State, Zip), (phone) (fax) and (E-mail) lines.
  12. Seller Signatures
    Two "Seller … Date" rules side by side for up to two sellers (borrowers). All are e-sign tabs placed when the form is sent; no broker signature line on this revision.
