The TXR 1413 form is organized into the following sections:

  5. Payment Information
    Requests detailed payment breakdown including schedule, amounts, and escrow components.
  6. Prepayment Information
    Requests information about prepayment penalties and any available waivers.
  7. Assumption Information
    Requests loan assumption details including transfer fees, buyer qualification requirements, and rate/payment changes.
  8. Insurance Information
    Requests property insurance details including coverage types, premiums, and agent contact information.
  9. Tax Information
    Requests property tax details including amounts last paid and breakdown by taxing authority.
  10. Other Information
    Provides an open-ended request for any additional relevant mortgage information.
  11. Return Information
    Directs where the lender should send the mortgage information response.
  12. Seller Signatures
    Provides signature blocks for up to two sellers to authorize the information request.
