The TXR 1411 form is organized into the following sections:

  1. Concerning the Property Located At
    One line under the heading "CONCERNING THE PROPERTY LOCATED AT" — the address of the property the keybox goes on.
  2. A. Listing Statement
    "The owner of the above-referenced Property has listed the Property for sale or lease with: ____ (Broker)."
  3. B. Owner Authorization
    Confirms the owner has authorized the Broker to place a keybox on the Property. No blanks.
  4. C. Keybox Description and Risk Disclosure
    Describes what a keybox is, explains that it allows entry in the tenant's absence, and warns of risks including unauthorized entry, theft, and property damage. No blanks.
  5. D. Tenant Authorization
    "The undersigned tenant authorizes Broker to place a keybox on the Property." No blanks.
  6. E. Special Provisions
    Space for additional terms such as showing time restrictions or notice requirements. The paper prints no rule here — the text is written into the white space under the heading (about seventeen lines).
  7. Valuables Notice
    "Owner and Broker advise tenant to remove or secure jewelry, prescription drugs, and other valuables." No blanks.
  8. Tenant Signatures
    Two "Tenant … Date" rules side by side, each with a "Phone:" and an "E-mail:" rule under it. The signature rules and their Dates are e-sign tabs placed when the form is sent; there is no printed-name, owner or broker line on this revision.
