Form Outline

The TREC 58-0 form is organized into the following sections:

  1. Header
    Identifies the form as approved by the Texas Real Estate Commission.
  2. Title Examination Advisory
    Advises the prospective buyer to either have a title abstract examined by their own attorney or obtain title insurance before purchasing the property.
  3. Utility District Notice Advisory
    Notifies the buyer that properties in utility districts (MUDs, WCIDs, etc.) carry special tax obligations and the seller must provide a statutory notice about district taxes and bonded indebtedness.
  4. Public Improvement District Notice Advisory
    Alerts the buyer that properties in public improvement districts (PIDs) carry additional assessment obligations and the seller is legally required to provide written PID notice.
  5. Date and Broker Identification
    Captures the date the notice is given and identifies the brokerage and the specific broker or sales associate providing the notice.
  6. Buyer Acknowledgment of Receipt
    The buyer acknowledges receipt of the notice by signing. Space is provided for up to two buyers.
